As a courtesy we are happy to assist you in obtaining the maximum
benefits specified in your contract. Most insurance plans are not meant to be a "pay-all" and you will
typically have a deductible and/or co-pay for services preformed. Your
insurance policy is a contract between you and your insurance company. Deductibles and estimated co-pay
amounts are due in full the day treatment is rendered. We accept most
insurance plans. Contact us to find out if we are a participating
provider for your policy. Even if we are not a participating provider
most PPO Plans offer out-of-network benefits. We do not accept DMO
insurance

is making the cost of optimal care as easy and manageable for our patients
as possible. ALL DENTAL FEES, CO-PAYS AND DEDUCTIBLES ARE DUE AND PAYABLE
THE DAY OF SERVICE. We are pleased to offer you the following payment

FULL PAY CASH DISCOUNT: We offer a 5% accounting adjustment to
patients who pay in full with cash or check at the start of their treatment.
